'Clinics need to have separated budgets'- Dr June M. Samuel

-BVIHSA CEO says discussions with Health Minister to rectify this has begun

CHURCH HILL ROAD, Virgin Gorda, VI- The need for clinics across the territory to have their separate budget allocation was one of the topics of discussion during a community meeting at St Mary’s Episcopal Church Hall in Virgin Gorda on Saturday, September 6, 2024.

The community meeting came just eight days following the tragic death of Mr Revovion A. Sprauve aka 'Bucky'. Mr Revovion was unable to access medical attention in the night at the Nurse Iris O'Neal (NIOC) where 24-hour operations had seized. Mr Revovion's family members were also present at the meeting.

Clinics do not have separate budgets- Dr Samuel reveals

Interim Chief Executive Officer of BVIHSA, Dr June M. Samuel said the BVIHSA has an overall budget of 42 million dollars; however, clinics do not have separate budgets.

“That is something that was brought to his attention, just this week we had that conversation about the fact that the clinics need to have separated budgets,” she stated.

Dr Samuel did say the focus of the BVIHSA is ensuring Virgin Gorda receives the comprehensive services provided out of the NIOC.

NIOC has challenges, most services available

The CEO said the clinic does have some challenges, including damage to the air conditioning following the passage of Tropical Storm Ernesto in August.

While this is being addressed, she said the staff will continue to provide services. Patients are able to access lab and x-ray services at the clinic as well as primary care services by physicians and nurses.

The clinic’s 24-hour operations were reinstalled last week and so a nurse is present to deal with any issues after 4:30 PM.

Resumption of specialist services at NIOC

Dr Samuel informed residents that they are looking to resume specialist services which were once offered at the NIOC on a visiting basis.

“We know that there has been a gap in that and we are looking at re-establishing those services that would have been visiting in the past,” she said.

Brochures were also handed out at the community meeting and Dr Samuel encouraged the residents to pay attention to the information on it. She said this includes all services available at the NIOC on a regular basis, in addition to emergency numbers.

      Thank god you don’t and will never have any medical issues you and your family. Those girls ain’t concerned with haters like you and the sister already stated publicly she ain’t running again. You all stay there suffering cause them girls are doing what they got to do to survive and live. Hate can never prosperity.     The best thing for all on the panel Saturday night to do is to just resign. You are all failures and have failed and still failing the system. All of your incompetence is the reason the BVI is not moving. Youall are too much into yourselves and your feelings and stagnating the growth of the healthcare system. If you know that clinics should have separate budget, why are you not moving everything possible over the years to push for it. Until this can happen, why isn't the funds for clinics not incorporated into the entire HSA budget. Last time I heard, all other clinics were shut down because of electricity disconnection. Now tell me. Can you imagine an emergency out in East and a person cannot simply run in to the Long Look or East End clinic for help? This is not development. This is not advancement. We are practically moving down the hill backwards and only all of you dunce set of stuck up moo moo don't realise it. Why because youall think if the idea don't come from you, it is not worth it. All of you please put down the government key and go home. Minister and the entire rubbish BVIHSA board included.

    Separate budgets is a step in the right direction. The bigger issue is the government machinery moves too slow and is plagued with very poor leadership in the various departments. Along with the separate budget is the need for their own administrative body with individuals who know their business.

      The Chairman and CEO sitting at the head table are responsible for developing and spending the money themselves. The budget for the clinics is completely within their control. How disingenuous of them to suggest otherwise. The lady in the audience presented them with a simple schedule to cover the clinic 24 hours with the staff th y have right now and they wouldn’t listen. Shameful. Just shameful. Then to be sitting their smiling and condescending was so horrible.

    It is poor budgeting process and there are not unlimited budgetary resources, and critical services should get allocated a commensurate share of limited resources pie , No Ministry is going to get all its needs met in any fiscal year; it should get a commensurate share though. And getting a commensurate share/allocation require a structured budgeting process. Budgeting should start with the MoF issuing an estimate of revenues for the fiscal year. Then each ministry should develop a line item budget with top/above line of items on top of needs, if cuts are needed it will occur from the bottom up. In its line item budget, the Ministry of Health should develop an above the line budget for clinics on the outlying islands. A budget is an estimate of revenues and expenses, a fiscal road map. Consequently, each entity should have a ready list of items they propose for cuts. They cannot wait for MoF to ask they should maintain a shelf ready list of cut, ie, 5%, /0%, etc. As others have noted, healthcare should be the number 1 national issue not just in words but in actions, ie, resources,,management, etc.

      Seriously? The BVIHSA is a statutory body. If they expect the Ministry to do a line item budget for them the overpaid Board, CEO, and Financially Controller should all be sacked and the act repealed. It was plain to see that the Chairman, CEO, and Managers on the panel from the HSA were all clueless and out of touch with their own operation.

If HSa cannot manage as a statutory board and manage financially, then turn it back over to central government. They are all clueless as Iif you're asking them to design a ship.         @ Voter, does not the BVIHSA falls under the purview of the Minidtry of Health? Yes, and the ministry should ensure that it is developing a proper budgeting ensures it delivers quality services. Secondly, does not the BVIHSA receive approx $40M annually from government?

    Dr. Samuels is joking and her call for a separate budget for the clinics is a weak excuse. This looks more like a (mis)management issue to me. If the BVIHSA gets 42 million dollars from the Central Government, then it is for the CEO and the Financial Controller at BVIHSA to create mini budgets within that agency and allocate funds accordingly to the various areas. They could start by stop hogging all the money and resources in Road Town and cutting back on some of these unnecessary “Senior Staff” they have there. Get rid of some of the lazy Queen Bees so that they can fund and hire more productive workers. Try managing what you have better first. And the Minister is just as clueless.

    I have read the comments and came to the conclusion that a story has three sides. Your side, my side and the truth. In the medi al field there is a term known as prophylaxis, that is you try to put measures in place to prevent catastrophic health conditions. If many persons practice healthy lifestyle health complications will be minimized.
    Eating healthy foods, drink less alcohol, avoid smoking and yearly checkup with your doctors will be better for your health. I hope that every one that read this article will make a concerted effort to adopt a new life style and don't procrastinate with their health. Remember that prevention is better than cure.

    How can we have a chairman who knows nothing about health care. He is an accountant. That is the problem in the BVI we have people who are chairman but knows nothing about the job.

    As much as the Minister of Health is to be made accountable for the mismanagement and maladministration of the BVIHSA, he is not the one who is to be blamed directly for the deficiencies in operation of the BVIHSA. Educational fact; the BVIHSA is an Independent Statutory Body. They through their "Act" prevent the minister from interference or any direct engagement in the day to day operations of the business affairs, "HE HAS NO SAY", but then they rely on the minister when they need money. "How demure..." The fact of the matter is, the BVIHSA is lead by a Chairman who is responsible for leading the implementation of policies, and building stakeholder/shareholder relationship to the business for financial and operational growth and benefits, where the acting CEO is responsible for management of the overall day to day operations of the medical institution. The minister will not have a clue of what is happening in the confines of the BVIHSA unless there is some major issue, where he is then forced at the helm.

    The BVIHSA is financially deprived because they have poor management and highly paid executive managers, who have no modern negotiating or financial management skills and they continue to rely on the Ministry of Health like a suckling baby latched to its mother's breast, when they should be a self sustained company.

    The issue in the poor management and poor operational affairs of the BVIHSA comes from the Chairman and the CEO. If the CEO is not able to answer questions and based on what we saw during the press conference as she constantly said she was not aware of the issues that people spoke of, or she did not know, this speaks a lot to her ability to manage.

    It is a fact that the BVIHSA is in its dysfunctional state because the persons who are responsible for directly leading the operations of the institution is not capable of doing so and this is where the Minister of Health need to be given the "fail mark." He can see that the persons in the top leadership position do not have what it takes to run the facility, but instead of him moving a motion to reset and identify the appropriate management support to appoint in these positions, he there playing patter cake fishers man. If he chooses to jeopardize his political career, because he insist on acting incompetent and blind to what is in front of him, then let him do so. But he must know that he is doing so at the health, welfare and lives of people of the British Virgin Islands.

      I agree with everything you have said and you have certainly answered the questions that should be asked at the same time giving much needed education to the public about how the governmental system works. I do believe that you are disingenuous by not stating that this was a chairman and a board that the minister inherited from the Unity Government and considering that we recently made a one year anniversary for this new government with the deadline of completing the COI recommendation
      breathing down the necks of the ministers there was very little time for familiarization and acclimation to a new ministry. It is unfair to assume that he saw the lack of leadership in those top positions and did not make a move to reset and identify the appropriate management support in these positions when based on your post you know things dont work like that, espically with those statuartory boards. Let's be fair.

    loads of tax money can be found to pay foreign artists to come into the territory to sign for a few hours but we have issues sustaining the development of our communities
